For services we can’t provide ourselves, such as elder legal advice or estate planning, we offer referrals from our select network of trusted experts and professional service providers, providing introductions and warm hand-offs to ensure the highest quality of consistent, attentive, and uninterrupted care.

Our services include:

• Assessments of client (in home or facility)

• Counseling

• Interviewing and selecting caregiver agencies or individuals; ongoing coordination of these services

• Making and coordinating medical appointments

• Attending medical appointments and providing follow-up reports to other care providers

• 24-hour on-call services for emergencies such as meeting client at hospital

• Transitioning care for clients moving from independent to assisted living or from hospital stay to home
