Located in beautiful Upper Michigan, Jensen Case Management, LLC utilizes a team of professionals with over 20 years of nursing and social work experience to customize care needs and optimize quality outcomes.  Geographic coverage area includes the Upper Peninsula of Michigan and Northern Wisconsin. Other locations can be considered upon request.

Experienced nurse case managers manage your work-related injuries and auto medical claims located in rural Upper Peninsula of Michigan and Northern Wisconsin.

Our team of experts also are knowledgeable of regional and national resources. When local providers do not offer services, we will coordinate referrals for specialized treatment, assessments, and arrange transportation.

AGING LIFE CARE
Also known as “geriatric care management” Aging life Care is a holistic, client-centered approach to caring for older adults or others facing ongoing health challenges.

Jensen Case Managment provide clients and families, guidance and navigation of complex health care systems. We lead with actions that help with quality of care decision making and optimal life choices.

We aim to reduces worry, stress and time off of work for family caregivers. Our initial visit is complementary followed by the development of a care plan to establish needs and a plan of care.

We want to customize each individual plan and will provide resources.
